Service Description

Strata Integration organizes your health history into a coherent, readable timeline, making visible the patterns, gaps, and decision points that are difficult to see when records are scattered across multiple providers and years. This phase requires your active participation. We review your available medical records, laboratory reports, and medication lists, and conduct a structured interview to map your health journey across time. Structure Two virtual sessions. Session 1: Health history mapping. We clarify your timeline, review available laboratory results, and identify patterns worth discussing with your physician. Following Session 1, you complete a hair-based nutritional screening, an educational orientation tool that provides context for how lifestyle factors including nutrition, sleep, and stress have affected your general wellness. It does not replace laboratory testing. Session 2: Synthesis and forward organization. We bring your history, laboratory trends, and screening findings into one structured document, identify up to five Health Flags for physician follow-up, and build your personalized four-week lifestyle education framework. What You Receive A Health Data Baseline document including: a mapped health history timeline across providers; nutritional screening findings in plain language; up to five Health Flags with suggested physician questions; and a four-week lifestyle education framework tailored to your history.
